Berkeley Edge Conference Recommendation Form

Thank you for your interest in the Berkeley Edge Conference, November 5 - 7, 2015, at the University of California, Berkeley. Please complete a recommendation form for each student you are recommending. We encourage you to recommend students with whom you have had direct teaching or research interaction with and whom you anticipate will succeed in a doctoral program at Berkeley.

Please note that the Berkeley Edge Conference is an NSF-sponsored program for underrepresented minority groups. The NSF defines an underrepresented minority in science, mathematics, and engineering as African American, Chicano/Latino/Hispanic, American Indian, Alaskan Native, or Native Pacific Islander.

Recommendation Form Instructions:

  • All questions with an * must be completed. The information may be lost and the form may need to be completed again if it is submitted without completing all fields with an *.   • If this is not a student-initiated application, please refer the student whom you have recommended to the Berkeley Edge Program website for full instructions and access to the Student Application Form for completion by September 25, 2015. Each student must complete the online application and submit additional materials by September 25, 2015.
